*On the premium due date or on next following the date the insured attains age 80, if the insured's life insurance benefit amount is greater than $5,000, it will reduce to $5,000, with an appropriate adjustment in premium.
**Premiums are based on each covered person's gender and age at time of issuance and when the covered person attains a new age bracket.
GRADED DEATH BENEFIT: During the first two years of coverage, the benefit payable for death due to sickness will be the premiums paid plus interest.
